To our Patients Regarding Medicare Wellness

Our desire is for you to take health care services – that can reduce the jeopardy of your injury or illness. Medicare disburses for some wellness care. However, it won’t pay for all kinds of wellness care essential for you. We need you to be aware of your Medicare assistance and how we can help you acquire a maximum of them.

The term “physical” is often used to describe wellness care. But Medicare does not pay for a traditional, head-to-toe physical. Medicare does pay for a wellness visit once a year to identify health risks and help you to reduce them. At your wellness visit, our health care team will take a complete health history and provide several other services:

Broadcasts to perceive falling risks, depression, and other complications

An inadequate physical exam to check your weight, blood pressure, vision., and other effects considering your gender, activity level, and age

Suggestions for other healthcare services and vigorous lifestyle modifications

We will ask some questions regarding your health before the appointment; you may have to fill out a form. Below is the form; if you want, you can fill it out and take it with you at your medicare wellness appointment.

Medicare Wellness Questionnaire

New or prevailing health issues aren't entertained in a wellness appointment. It's a separate service and may need lengthier appointments. If you want doctor’s assistance with a health issue, a prescription fill-up, or something else, kindly inform our arranging staff beforehand. You need to pay separate charges for these services if you want them on the same day or a different than your wellness appointment.
